Exercise is another key component of maintaining a healthy self. Setting up exercise programs is essential for prevention of stroke and heart attacks which seems to have a correlation with increased dementia. We as health professionals have a responsibility to teach and assist people to maintain a lifestyle that will prevent some of the key factors which contribute to AD. Nurses and health care professionals are the first line to addressing these issues and assisting people to become aware of ways to keep and stay healthier during the golden years. Using the research that has been applied to best practice is the way that nurses can make a huge difference in their client’s lives.
The clients weight and appearance are an indicator of nutritional status, loose fitting clothing could indicate recent or sudden weight loss, eating habits and dental issues should be assessed, as well as who is preparing the meals. Other activities of daily living should be examined including grooming and hygiene, urinary continence should be assessed at this time, and how much assistance is needed. Sexual function and relationship status should assessed, if the client is sexually active it is important to determine if the client is monogamous and in a long term relationship, as STD’s are on the rise with seniors. (Jameson, 2011) A thorough assessment of the clients ability to perform daily activities should be done, questions should include if the client can walk without problems and how far (at some point during the exam the client should be observed to objectively assess gait), does the client exercise and if so how long and how often, and if there is pain during exercise. The cognitive assessment should also include a mood/depression evaluation; questions should include recent family or friend losses, sleeping patterns, spirituality, feelings of sadness or hopelessness, and social interaction.

The most common diseases affecting older people--cardiovascular and cerebrovascular diseases, diabetes, osteoporosis and cancer—are all related to dietary factors.
I would recommend that Mr. Bley utilize the senior center to get free hearing loss screening and education about his hearing loss and the dangers of not using his hearing aid. The Senior Center also provides help with smoking cessation, and education about how smoking can worsen emphysema and other pulmonary diseases. The writer relates to this patient, caring for this population daily. The focus of the care that will be provided in the future will now be more geared to understanding the patient. Caring for the patient not only physically, emotionally, and spiritually, but also socially.

This might be as a result of a medical reason such as diabetes, medication they are prescribed, allergies against certain foods like nuts, they may have to have a gluten free diet, or the patient may be under or overweight and require a specialist diet. In recognising our client’s right to have a choice in the food they eat we should ensure our clients maintain as an individual, and we should promote a positive mental health to our clients.

Physical health mean all the activities, work out, and behaviors to keep your body healthy. The physical fitness, nutrition, performing self exam are the most important to have a good healthy. As an important role in how you feel, emotional health can make a big change into your health, social skills and interpersonal relationships with others.
The concept of promoting health through preventive measures and the practice of good health habits is another way of saying: I want to maintain healthy and long life. Becoming aware of my current health status, and taking the necessary steps to maintain a balanced healthy life span determines my commitment in securing my present and future health. Many people do not focus on their health until they are sick or someone close becomes ill. It is very important for me to stay ahead when it comes to my health. One way that I promote my own wellness is adding more antioxidants to my diet such as garlic and ginger, fish and dairy products which has virus fighting properties and supports the immune system.
The primary focus on this level of care is to help ward off diseases through a healthier life. Patients can achieve this level of health through knowledge and understanding from education. Nurses can assist in the promotion of primary care by educating patients regarding the importance and benefits of vaccinations and avoiding risky behaviors that may lead to diseases, such as, smoking and unprotected sex. Nurses at the primary level may also educate patients regarding healthy eating and routine exercises to decrease risk factors. The secondary level of health promotion involves early diagnosis and rapid intervention to decrease the progress of any disease process.
