My level of communication within my job role is of extreme importance with regards to the client who should be at the heart of all communication around any establishment or company. Dealing with people with hearing impairments, disabilities, mental health issues to name a few, it is essential my communication is clear and not lengthy. Effective communication is what enables all the different departments to work well within the organisation including liaising with external agencies and individuals such as the CQC, Social Services, Continuing Health Care, Hospices and health professionals. It places the person at the centre of all - thinking, discussions, processes and procedures. The person-centred approach also sees the interactions between the person and different elements within their support systems as being of paramount importance, and therefore places emphasis on supporting the system that supports the person i.e. the family and friends, and other professionals. It discovers and acts on what is important to the person. In dementia care is a key aspect of best practice.
This means being involved in their assessments, care delivery and support planning, basically person centred values is making sure all approaches, policies and procedures and care practices put the residents at the centre of the day-to-day activities. It should also include the residents and their families in the planning and maintaining of this. 1.2 Explain why it is important to work in a way that embeds person centred values. The underlying purpose of person centred values is to ensure that the individual needing care is put at the very centre of the decision making progress about their life and the services and support they want, need and require. Person centred values is about putting an individuals needs and choices first, respecting their privacy and dignity and giving that individual as much independence as possible.
One of the most important communication systems used in the workplace is a person centred care plan and the risk assessments involved in this. The care plan, when reviewed regularly communicates a range of relevant information to all staff and allied health professionals, to enable them to contribute to the effective delivery of care to that individual. The support workers roles is too make sure they support the individual to there best of there ability and follow their care plan. It is the support workers role to provide the client with good quality of care. To do this you need to involve the individual in any decision making and encourage both there participation and feedback. The role of a care manager fulfils the role of a registered manager. The manager will be available to talk about any issues concerning the individuals 24 hours a day or if any problems occur.
1.2 The duty of care affects my work role on a day to day basis, I have to prioritise and often make decisions for client’s best interest. I must always carry out my duties that are in my job description. I follow procedure and provide a standard of care in line with the principle codes of practice in all aspect. I make have access to all resources and equipment that may assist me, and if I don’t I ask my manager. I observe confidentiality at all times, I observe and make sure I update my knowledge and skills on a regular basis; I understand the importance and have the confidence to air concerns, which may be delicate and involve not only work colleagues, but also people I support.
