Introduction
Concept of supervision includes facilitation and assessment which has been understood as an umbrella term of education, evaluation and mentoring (Butterworth & Fauquier 1992). Individualized supervision model is also known as mentoring which includes supervision and assessment by a personal facilitator. Saarikoski (2003) mentions that, mentoring in nursing is a learning experience between two people to achieve career progress from ones’ experiences. As a registered staff nurse working in a foreign country with 3 year experiences in the geriatric nursing. Having completed my diploma in year 2010 as recognized by the ministry of Health Allied in Malaysia. Facilitated learning and assessment is being carried out in my geriatric medicine department. Whereas, allocation of a preceptor to a Year 2 diploma in nursing student is arranged by nurse manager. Nursing student in their 2 year of clinical practice needs to achieve clinical skill (also other soft skills e.g. judgment) competency during clinical placement. During her one week placement in the geriatric department, Julie (not real name) was assigned to me for supervision and facilitation of her clinical placement. Her learning progression was recorded and evaluated her skill assessment. Guiding and monitoring Julie during her clinical placement, she was noted to be a proactive nursing student {Appendix 1]. Before the learning phase, a learning contract was being drawn out between Julie and me [appendix 2].
This clinical placement consists of supervision of Julie on care of nasogastric tube feeding.

Before the start of her clinical placement, Julie had been taught of nasogatric tube care during her practical lessons. As staff assign to supervise Julie in this skill I have ensure that Julie has basic knowledge of nasogastric tube before guidance being carried out.
