EMRs come with instant messaging so if another member of the medical staff has a question then they can communicate with each other through the instant messaging without having to track down the other person. Instant messaging allows for someone to receive the answer they need as if they were having an interpersonal conversation. What are some advantages and disadvantages of EMRs? Response: I am currently working in a neuroscience intensive care unit and would like to explore how my facility could expedite the diagnosis and treatment for patients presenting with stroke-like symptoms. In our unit, it has been drilled into our heads that “time is brain,” but unfortunately the rest of the facility does not respond so promptly when there is a new onset of stroke symptoms. Today many facilities have already implemented “Rapid Response” or “Code Team” nurses to respond immediately when a patient develops cardiac or respiratory arrest. Using this concept of calling a “code” when a patient develops stroke symptoms expeditse the diagnosis and treatment which would allow patients to have the best possible prognosis following a stroke (Lu Daly, Orto, & Wood, 2009). My PICOT question: Does initiating a rapid response nurse when patient presents with stroke like symptoms allow for more timely treatment, within the first 30 minutes from the onset of symptoms, compared to having patients wait to be evaluated by a physician?
These computer based systems are used to direct nurses in the clinical decision-making process. The software is designed to identify specific patient characteristics, such as weight and drug allergies, and to match them to a computerized clinical database that includes information such as safe medication ranges and lab values that require verification prior to medication administration (Jennifer B. Lomoine & Helen M. Hurst, 2012). The way this information was expressed in this article was very informative. Medication errors can happen at any time, but I never thought there were so many errors in administering medication to infants. After reading the article I would agree that the technology of smart pumps would help in reducing medication dosing error.

It enables rapid identification of at-risk patients and access to their medical history, thereby enabling rapid diagnosis and treatment especially in emergency situations. EMRs threaten our privacy. In this day and age when people's mantra is "I need my privacy", not many people are comfortable about having their entire medical history recorded and digitized for almost just anybody to see - in other words, incursion into people's privacy.
On the surface, just being able to simply send the basic patient chart to another doctor in the practice, the front desk, the nearest hospital or even a specialist is an improvement on the current paper process (Source One). However, even more important than ease of use, is the ability of the EMR software to track patient systems across multiple providers in the same practice or across hospital in an entire integrated health network (Source Seven). The physicians would have the option to view other patient charts with similar patient symptoms to a patient they were currently treating in real-time, which would allow them to see what they were ultimately diagnosed with, potentially saving the life of the patient on the table. Money can become a big issue with compliance, but money is not the only reason why elderly are non-compliant with their medications. Family support, understanding their diagnosis or diagnoses, unaware of resources, and no discharge plan can all add to the stress causing noncompliance. Fear for the elderly is not uncommon, so without family support this fear can be even greater. Family involvement as well as understanding is critical in compliance for the patient. Nurses and doctors need to involve the patients and family members in their care from day one.
