This in turn exposes not only patients, but also nurses to a significant amount of noise and alarms, ultimately leading to the clinical problem called alarm fatigue. As defined by the Joint Commission, alarm fatigue is known as the desensitization of medical staff as a result of sensory overload. This overload ultimately results in a delay of an alarm being answered, and sometimes someone completely missing the alarm altogether (The Joint Commission, 2015). Alarm fatigue has been recognized as a contributing
Primary care nursing is a standard practice in the intensive care unit, and by nature is a proponent of transformation. Patients benefit from the evolution of nurses obtaining higher levels of training and education. For example, a nurse in the intensive care unit through vigilance and training can affect incidence of ventilator acquired pneumonia and central venous catheter infections. Hospital induced delirium is a very expensive add-on to the cost of healthcare, but through early detection and treatment can be minimized by the bedside nurse. Essentially, if nursing performs at the peak of their training and education we can greatly impact in every area the goals of the IOM report.
Critical Incident reflection in Midwifery Practice Midwife practitioners are faced with complex decision-making and problem-solving activities on a daily basis. Benner (1984) believes that there is a wealth of untapped knowledge in clinical experience. Schon (1983) recommends reflection on critical incidents as a valuable learning tool. The value of reliving and analysing clinical incidents has enormous potential. Benner (1984) used clinical situation examples (exemplar's) to show how 'expert' practitioners can positively influence outcomes for clients.
This can be attributed to increase demands on nurses to produce more because there overworked coworkers have increased use of sick leave related to burnout. Patients and family members are beginning to realize the inadequate quality of health care services administered as the nurse is often very tired as the nurse to patient ratio surpassed safe patient care levels. The supply curve emphasizes change, allowing the health care industry to focus on a range of solutions indication how they will fix the shortage as the demand increases (Getzen, 2007). “The major factors and trends behind the growth in RN demand include: population growth, aging of the population, increased per capita demand for health care, and trends in health care financing,” (Bureau of Health Professions, 2004,
Once again the shortage in nursing will be a problem that affects this area as well. Quality care is the highest priority to NYM. It is apparent that they do everything they can to make quality care #1. Again comes the issue with the nursing shortage. Having such a strain on nursing makes unhappy nurses and it will show in patient care.
There is a general consensus among all the articles that I read regarding the need for more studies, tools to assess and programs at hospitals for nurses. “An increased awareness of the emotional demands facing today’s nursing workforce is of utmost importance” (Erickson & Grove, 2007). Nursing involves empathic relationships with patients, the empathic level of caring leaves us vulnerable for compassion fatigue. Compassion fatigue can affect nurses across the spectrum, from students to experienced nurses. The number of students it affects has yet to be studied, but as read in the Nursing Standard, “Nursing students in the US are being taught about compassion fatigue to help them cope with stress”("Compassion Fatigue Hits," 2011, p. 7).
Many of these infections continue to occur more, especially in the intensive care unit (ICU) and other departments. From the six aims of improvement according to the Institute of Medicine (IOM), central line infection affects patient safety, effectiveness, and patient centeredness. Patient safety is important because it is dealing with the patient’s health status. When a central line infection occurs, whether if it starts with an infection or leads to death, patient safety is on the line. Effectiveness affects the healthcare quality because with the science and knowledge needed to prevent CLABSI, it continues to occur more than it should.
The media is very influential in regards to how society perceives nurses; this is something which has altered the image of nursing within the past, (Benison and Voogd, 2008) Appropriate examples of cases which have been focused upon within media will be underlined and discussed throughout this essay. Nurses must have a significant degree of self-awareness in order to deliver patient centred
According to the literature reviewed, there is a direct correlation between turnover of nurses and CNAs to the quality of patient care. Hunt (2009) gives a concise explanation of the correlation of staff shortage on patient care: Staff shortages caused by nursing turnover are associated with significant decreases in the general quality of patient care, increases in the length of patient stays within hospitals, and greater numbers of hospital-acquired patient illnesses and conditions that do not qualify for Medicare/Medicaid reimbursement (p.3) Another report published by the American Association of College of Nurses’ stated the same thing that there is a correlation between nurse to patient ratio and the quality of care given. As a result many organizations are left having to do major damage control externally, to prevent a bad reputation. Instead of external PR, organizations need to start taking a hard internal look at their staff management. Reasons for Staff
According to the Nursing and Midwifery Council (2008) research is essential to nursing field to enhance evidence based practice. Melnyk and Fineout-Overholt (2005,p339) define evidence based practice as a problem solving approach that integrates a systematic search for and critical appraisal of the most relevant evidence to answer burning clinical questions while maximizing quality care and limiting cost. However, evidence based practice in nursing is more challenging than its use in the medical profession because of the multiple sources of nursing knowledge (Carrier andBrassey2006,p40). For this reason, it is important for researchers to understand how to figure out research techniques appropriate for them coming up with the best evidence on various issues being handled. Clearly-holdforth( 2008, p43) advocates the use of PICO question framing format while researching for it is a standardized criteria for determining a research question for all methodologies although different methodologies tackle the research
