A community is a group of people who live within the same geographic location and share the same type of lifestyle, values and norms. Community health then seeks to look at the general health beliefs and practices within that community. According to Stanhope and Lancaster (2008), “Community health is defined as the meeting of collective needs through identification of problems and management of behaviors within the community itself and between the community and the larger society” (p. 370). Community health is not limited to improving outcomes of an individual suffering from a particular disease or illness, but concentrates on the improvement of the community as a whole.

Training of all health care staff will be provided by Angela D Gay RN, Program Developer. The Director of the Urban Ministries Clinic will remain giving oversight and being overseer. The senior staff nurse/charge nurse will be responsible for assuring patients are being screened for communicable diseases and that follow appointments are being made to continue success within the community. One of the participating nurses will be selected to collect and tally the quality of the questionnaires on an ongoing basis. As each client comes into the clinic they will sign in and complete history.
You as a carer have a duty of care to keep the individual safe but you must always respect the individuals’ rights and choice, so this would cause a dilemma. For example if an individual no longer wishes to use her walking frame, but her care plan states she needs it to move around then you must ensure you encourage her to use it. In this situation you could carry out a risk assessment to ensure it is dealt with as safely as possible. You would need to explain the risks to her and make sure she understands. You could come to a compromise and suggest she uses a walking stick for a while instead to see if she can manage and in the meantime you can monitor the situation.
