Reflection of the "P" Family of Bellmore Community Assessment Rex, L. NUR/405 January 27, 2013 Prof Beth Edwards, BSN, MSN Reflection of the "P" Family of North Bellmore Community Assessment Community and public health nursing (PHN) involves working with communities and populations as equal partners, and focusing on primary prevention and health promotion (ANA, 2007). In the past decade, public health nurses practice in a variety of settings including, but not limited to, community nursing centers; home health agencies; housing developments; local and state health departments; neighborhood centers; parishes; school health programs; worksites and occupational health programs. The high-risk or better known as vulnerable populations are often the main focus of care. This population may include the frail elderly, homeless individuals, sedentary individuals, smokers, teen mothers, and those at risk for a specific disease. Community solutions can have widespread influence on health and illness patterns of many levels of clients including individuals, families, groups, neighborhoods, communities, and the broader population (ACHNE, 2003).
