Diagnostic In this case study Nick was recommended that he had a psychiatric evaluation because he broke down and cried in his routine physical examination. This break-down, according to Nick was due to the fact that he was depressed and was thinking about a former suicide attempt when he was a teenager. I however, through the reading of his case study disagree that clinical depression is what Nick’s diagnostic should be. There are many clues that helped me determine that Nick’s mental disorders should have the most emphasis under the Axis II of the DSM-IV-TR. This Axis for the DSM-IV-TR deals with underlying pervasive or personality conditions, as well as mental retardation, which includes personality disorders. I believe that Nick indeed has not one but possibly two personality disorders. He has co-morbidity with Histrionic Personality Disorder and narcissistic personality disorder. There are many reasons why I chose histrionic personally disorder. First off I notice that his interaction with others is sexually inappropriate, which is one criterion for Histrionic Personality Disorder in the DSM-IV-TR. When he first met with his therapist, he told him that he wished he was as attractive as his family doctor. This is not an appropriate way to behave when meeting someone in this sort of situation. Especially since it was one of the first things said to the therapist. Not only is this behavior deemed sexually inappropriate but Nick is considering his relationship more intimate than it really is. Because he was being more frank and open with his doctor than he should have been, and he was more intimate than he needed to be, his behavior then could be considered another criterion for Histrionic Personality Disorder.
